• Is NOT a nursing home.

• Enforces that the relationship with your provider – your doctor’s office and your health care system – is the key to quality care. Your doctor will work with you to identify your health care needs and goals, answer your questions and connect you to specialized care if you need it.

• Coordinates your care if additional services are needed outside what is provided by your medical home.

• Offers preventive care as well as treatment for disease or chronic conditions. Quality care means your doctor considers all aspects of your health, including your diet, physical activity, health history and your family’s health history.

• Maintains your medical record, sharing it with specialists as needed, protecting your privacy, and updating it to reflect the current state of your health and the care you receive, including office visits, medications, tests, hospitalizations and consultations with specialists.
